Each Custom Form will store responses. These responses can be exported to a .csv file.

Forms can be filtered based on Paid or Unpaid status.

If the form includes payment options, the following will be included:

• whether the invoice is paid
• the amount of the invoice.
• a link to the invoice (based on permissions)

Controls are available to delete a specific entry. Entries with paid invoices can not be deleted.

Form field editing: text input fields, radio button questions, pull down questions can be edited inline.  In addition, checkbox input fields can also be edited by entering the form submission using the “Show form response” menu item for the specific submission.

Editing fields in forms is often leveraged for admin only questions. Note, fields from player select and user select fields can not be edited.

Warning: editing a form field that has a payment implication does not modify the invoice that has already been paid. Any payment impact should be noted in the invoice and the payment change will need to be handled separately (e.g., additional charges apply).

Export of any custom form based workflow (e.g., Custom Form, Tryout, Form Registration, Data Collector) that includes a player will include a Player ID field in addition to a User ID for the submitter. This helps to correlate to the original player and user even if their name changes.

Warning: removing a form field will remove the associated data.

Invoice Management

Byga provides capabilities to manage the Custom Form invoices. Invoice Management is documented separately in the Byga Invoice Management Module document.

USA Lacrosse Membership Verification

USA Lacrosse provides a service that enables verification of a player’s current membership status. Byga provides this option with a USA Lacrosse verify form widget that can be added to any form-based workflow (custom forms, form based registrations, tryouts and data collectors). This can be used to verify the status of new players in the system as well as existing players.

An option is available to require the verification to be valid through the date entered. For example, if a camp will be organized for June 24-July 1, the membership can be specified to be valid through July 1.

The form submitter will be required to submit the membership ID number that matches the selected player information in order to proceed and submit the form.
